The Entry/Registration forms are available here as a single PDF file. If you would like to receive a hard copy by mail, please contact us.

No registration will be considered complete until all forms (the Entry Form,  Medical Consent Form, and Waiver Form) have been received (completely filled out) along with payments.

Deadline: Forms must be postmarked on or before July 30, 2008 to avoid a $20.00/competitor late fee.
